60-Day Notice of Proposed Information Collection: Statement Regarding a Valid Lost or Stolen U.S. Passport Book and/or Card
ACTION:
Notice of request for public comment.
SUMMARY:
The Department of State is seeking Office of Management and Budget (OMB) approval for the information collection described below. In accordance with the Paperwork Reduction Act of 1995, we are requesting comments on this collection from all interested individuals and organizations. The purpose of this notice is to allow 60 days for public comment preceding submission of the collection to OMB.

The Department will accept comments from the public up to September 13, 2024.

Abstract of Proposed Collection
[top] The Secretary of State is authorized to issue U.S. passports under 22 U.S.C. 211a et seq., 8 U.S.C. 1104, and Executive Order 11295 (August 5, 1966).
Methodology
Passport bearers may submit the form on the Department of State's website, www.travel.state.gov, where it can be completed, signed, and submitted electronically. The DS-64 is also available at eforms.state.gov where it can be completed online and printed for signature and submission. Additionally, passport bearers have the option to call the National Passport Information Center (NPIC) at 1-877-487-2778 or mail in a hardcopy of the form. The form can be obtained at any passport agency or acceptance facility.
